ObjectivePatient involvement is used to describe the inclusion of patients as active participants in healthcare decision-making and research. This study aimed to investigate incoming year 1 medical (MBChB) students’ attitudes and opinions regarding patient involvement in this context.MethodsWe established a staff–student partnership to formulate the design of an online research survey, which included Likert scale questions and three short vignette scenarios designed to probe student attitudes towards patient involvement linked to existing legal precedent. Incoming year 1 medical students (n=333) were (...) invited to participate in the survey before formal teaching commenced.ResultsSurvey data (49 participants) indicate that students were broadly familiar with, and supportive of, patient involvement in medical treatment. There was least support for patient involvement in conducting (23.9%), contributing to (37.0%) or communicating research (32.6%), whereas there was unanimous support for patients choosing treatment from a selection of options (100%).ConclusionIncoming members of the medical profession demonstrate awareness of the need to actively involve patients in healthcare decision-making but are unfamiliar with the utility and value of such involvement in research. Further empirical studies are required to examine attitudes to patient involvement in healthcare. (shrink)

We examine George Boolos's proposed abstraction principle for extensions based on the limitation-of-size conception, New V, from several perspectives. Crispin Wright once suggested that New V could serve as part of a neo-logicist development of real analysis. We show that it fails both of the conservativeness criteria for abstraction principles that Wright proposes. Thus, we support Boolos against Wright. We also show that, when combined with the axioms for Boolos's iterative notion of set, New V yields a system equivalent to (...) full Zermelo-Fraenkel set theory with a principle of global choice. This advances Boolos's longstanding interest in the foundations of set theory. (shrink)

Is the pool of fair predictive algorithms fair? It depends, naturally, on both the criteria of fairness and on how we pool. We catalog the relevant facts for some of the most prominent statistical criteria of algorithmic fairness and the dominant approaches to pooling forecasts: linear, geometric, and multiplicative. Only linear pooling, a format at the heart of ensemble methods, preserves any of the central criteria we consider. Drawing on work in the social sciences and social epistemology on the theoretical (...) foundations of the wisdom of crowds, we explain how our observations present an exception to the general trend of finding tradeoffs between the accuracy and fairness of forecasts. My dissertation consists of two main parts. In the first part, I begin by assuming the plausibility of the libertarian thesis that agents sometimes could have done otherwise than they did given the very same history of the world. In light of this assumption, I undertake to develop a model of agency which does not employ the concept of agent-causation. My agency theory is developed in three main stages: I suggest that any agency theory must satisfy four desiderata: It must (...) adequately account for the freedom and responsibility of human agents. It must provide an adequate answer to the question of what distinguishes human actions from mere happenings. It must adequately account for the epistemological fact that human agents have an immediate and nonobservational awareness of their actions. It should have the support of a respectable philosophical tradition. I argue that agent-causation provides a theory of agency which fails adequately to satisfy these four desiderata. I claim both that human actions are uncaused exercisings by agents of their powers and that human agents typically act for reasons. I maintain that the central issue dividing agency theorists and nonagency theorists is whether reasons for performing actions cause the latter. I maintain that the sense of 'because' in 'I did x because...' is not causal in nature, but teleological. ;Having developed my model of agency, in the second part of my dissertation I investigate what are some of the paradigmatic actions human agents perform. I claim that bodily actions, such as moving an arm, are psychological in nature. Essential to my argument is the concept of a body-image. A normal agent 'feels' as if she is present in her physical arm. The way a normal agent feels is revealed in the case of a phantom limb. The subject of a phantom arm feels as if her amputated physical arm is still there and she can move while feeling armishly. I maintain that the bodily action of moving an arm is the agent's movement as a psychological arm-image of her physical arm, whether the physical arm is present or not. (shrink)

Debate concerning virtual reality is often drawn in terms of sharply defined dichotomies--for example, between "real" (or "actual") and "virtual," "authentic" and "inauthentic," and "natural" and "artificial." In this paper we offer an alternative approach by suggesting a conception of a virtual world that highlights a continuity and commonality with our sense of everyday reality. We accomplish this in part by an examination of the English picturesque garden as if it were a virtual world partially constructed out of ideas and (...) objects collected during travels to foreign lands on the Grand Tour. Such foreign travel transformed not only the English person's sense of self, but also altered the English landscape. We conclude that in one sense the "real" England is also a "virtual" reality. In the face of an impossibility result, some assumption must be relaxed. The Mere Addition Paradox is an impossibility result in population ethics. Here, I explore substantially weakening the decision-theoretic assumptions involved. The central finding is that the Mere Addition Paradox persists even in the general framework of choice functions when we assume Path Independence as a minimal decision-theoretic constraint. Choice functions can be thought of either as generalizing the standard axiological assumption of a binary “betterness” relation, or as providing (...) a general framework for a normative (rather than axiological) theory of population ethics. Path Independence, a weaker assumption than typically (implicitly) made in population ethics, expresses the idea that, in making a choice from a set of alternatives, the order in which options are assessed or considered is ethically arbitrary and should not affect the final choice. Since the result establishes a conflict between the relevant ethical principles and even very weak decision-theoretic principles, we have more reason to doubt the ethical principles. (shrink)

Saul Kripke once noted that there is a tight connection between computation and de re knowledge of whatever the computation acts upon. For example, the Euclidean algorithm can produce knowledge of which number is the greatest common divisor of two numbers. Arguably, algorithms operate directly on syntactic items, such as strings, and on numbers and the like only via how the numbers are represented. So we broach matters of notation. The purpose of this article is to explore the relationship between (...) the notations acceptable for computation, the usual idealizations involved in theories of computability, flowing from Alan Turing’s monumental work, and de re propositional attitudes toward numbers and other mathematical objects. What is the potential for improvements in the functioning of consciousness? The paper addresses this issue using global workspace theory. According to this model, the prime function of consciousness is to develop novel adaptive responses. Consciousness does this by putting together new combinations of knowledge, skills and other disparate resources that are recruited from throughout the brain. The paper's search for potential improvements in consciousness is aided by studies of a developmental transition that enhances functioning in whichever domain it occurs. (...) This transition involves a shift from the use of procedural (implicit) knowledge to declarative (explicit) knowledge. However, the potential of the transition to enhance functioning has not yet been realised to any extent in relation to consciousness itself. The paper assesses the potential for consciousness to use declarative knowledge to improve its own functioning and to thereby enhance human adaptability. A number of sources (including the practices of religious and contemplative traditions) are drawn on to investigate how this potential might be realised. (shrink)

This book, the manifestation of their work, is a critical examination of the impact of the NCLB on the lives of children, families, and teachers.

This Article presents an innovative institutional strategy for global climate protection, quite distinct from, but ultimately complementary to and supportive of the currently stalled UNFCCC climate treaty negotiations. The bottom-up strategy relies on a variety of smallerscale transnational cooperative arrangements, involving not only states but sub-national jurisdictions, firms, and CSOs, to undertake activities whose primary goal is not climate mitigation but which will achieve greenhouse gas reductions as an inherent byproduct. This strategy avoids the inherent problems in securing an enforceable (...) treaty to secure the global public good of climate protection by mobilizing other incentives - including economic self-interest, energy security, cleaner air, and furtherance of international development - to motivate such actors to cooperate on actions that will also benefit the climate. These bottom-up regimes will contribute to global climate action not only by achieving emissions reductions in the short-term, but also by linking the bottom-up regimes to the UNFCCC system through greenhouse gas monitoring and reporting systems. In these ways, the bottom-up strategy will help secure eventual agreement on a global climate treaty. (shrink)

Hegel is known for coining the word “phenomenology” as a description of the methodological approach that he pursues in the famous work that bears this title. It has long been an open question the degree to which the later philosophical school of phenomenology in fact follows the actual method developed by Hegel or if it merely co-opted the name and applied the term in a new context. While Husserl was dismissive of Hegel, the French phenomenologists were generally receptive to Hegel’s (...) conception of phenomenology. This chapter argues that there are in fact some important points of continuity and that the French phenomenological school’s understanding of Hegel as a forerunner of their movement is quite legitimate. In the UK, over 70% of AIDS, including new cases, is located in a few Districts in central London where the distribution of previously occurring and new cases is essentially confined to the original risk groups of homosexual/bisexual men, drug addicts of both sexes, and some of their sexual partners and consorts. But control policy is still based on the assumption that HIV has already spread from persons in these risk groups into the general population, and that it will spread (...) hereafter at an increased rate because of heterosexual transmission to cause a widespread epidemic of AIDS.The basis and implications of this policy were investigated in the South East region adjacent to London, Analysis of demographic and epidemiological data shows that, with one exception, there is very little extension of AIDS from affected Districts in London to the surrounding region or even to the suburban fringe. Where AIDS is prevalent, as in this exception, the distribution follows the same, original pattern which relates essentially to risks arising from life-styles. There is no significant association between the prevalence or spread of AIDS and conventional markers of deprivation, economic and social disadvantage.Allocations of personnel, services and expenditure, assessed from returns required under the AIDS Control Act of 1987 and official registration data, continue to follow the original policy assumptions. They are therefore unrelated to the numbers of existing and new cases, disproportionate and unrealistic. Claims that this widespread excess of effort is justified by the fact that AIDS has not spread to the general population are falsified by the continuation of cases almost exclusively in risk groups. There is nothing in the data required under the Act or in registrations in this main locus of AIDS in the UK to suggest any change in this or to justify continuation of current expenditure and redundant activities.There is no evidence in these data that ethnic variations in the resident populations of Districts are associated with variations in the prevalence of AIDS. However, the larger figures available in some of the national data do indicate a disproportionate increase in some minority ethnic groups. Further detail about risks factors in these groups are required and, meanwhile, effort and expenditure should be re-orientated toward treatment, contact tracing and other public health measures for more effective containment of the continuing spread of AIDS in all the high risk groups.
